Informed Consent Template

For therapists to provide to their clients before entering client data into the Platform

Instructions for Therapists

This template is provided for you — the therapist, psychologist, coach, or counselor — to obtain informed consent from your clients before entering their data into AITACS CRM. It fulfills the requirements of HIPAA Authorization, GDPR Article 9(2)(a) (explicit consent for special category data), and professional ethical standards.

How to Use This Template

Step 1: Customize the form below with your practice name and contact details (replace the bracketed placeholders).
Step 2: Print the form or present it digitally.
Step 3: Review each section with your client verbally, ensuring they understand the role of AI in your practice.
Step 4: Have the client sign. Keep the signed copy securely (encrypted digital or locked physical storage).
Step 5: Only after consent is obtained, enter the client's data into the Platform.

Customization

Fields marked with [BRACKETS] must be replaced with your specific information before use. You may add additional clauses required by your jurisdiction, licensing body, or institutional policy. If you modify the AI-related sections, ensure they remain accurate to the Platform's actual data handling as described in the Privacy Policy.

Additional Notes for Practitioners

Storing Signed Consent

Store the signed consent form securely — either as an encrypted digital file (scanned PDF on an encrypted drive) or a physical copy in a locked filing cabinet. Retain consent forms for the duration of the therapeutic relationship plus the retention period required by your jurisdiction (typically 5–10 years after the last session). If you use the Platform's built-in consent tracking fields, these record the fact that consent was obtained but do not replace the signed form.

When to Re-obtain Consent

You should obtain updated consent if: (a) there is a material change to how the Platform processes data, (b) a new AI provider is engaged, (c) the scope of data shared with AI changes, or (d) the client's treatment modality changes in a way that affects data handling. AITACS CRM will notify you of any material changes at least 14 days in advance.

Handling Opt-Outs

If a client opts out of AI processing (Section E, Right 4), continue using the Platform for record-keeping, scheduling, and session notes — simply do not use the AI analysis feature for that client. You can do this by not initiating AI analysis for the client's record. The client's data will remain in the CRM but will not be transmitted to OpenAI.

Translations

This template is provided in English. If your clients require the consent form in another language, you are responsible for providing an accurate translation. Ensure the translation preserves the meaning of all AI-related disclosures and the client's rights as described above. The translated version should be reviewed by a qualified professional before use.

Common languages for which translation may be needed: Hebrew (עברית), Russian (Русский), Arabic (العربية), Spanish (Español), French (Français), German (Deutsch), Portuguese (Português), Chinese (中文).
